摘要:
目的 探究青少年及成人特应性皮炎患者疾病严重程度与血清25-羟基维生素D、总IgE和嗜酸性粒细胞计数的相关性。方法 参考SCORAD评分法评估112例青少年及成人特应性皮炎患者疾病严重程度,并检测患者及70例健康组血清25-羟基维生素D水平以及患者总IgE、嗜酸性粒细胞数计数。结果 特应性皮炎组患者血清25-羟基维生素D水平(20.42±6.96)ng/mL明显低于健康组(28.68±7.85)ng/mL,差异有统计学意义(P=0.000<0.01)。重度患者血清25-羟基维生素D水平(18.93±7.06)ng/mL低于轻中度患者(21.62±6.70)ng/mL,差异有统计学意义(P=0.041<0.05); 重度患者总IgE水平(5 184.08±7 533.82)IU/mL明显高于轻中度患者(1 075.07±1 777.37)IU/mL,差异有统计学意义(P=0.000<0.01); 重度患者嗜酸性粒细胞计数升高(37/50)的比例明显高于轻中度患者(17/62),差异有统计学意义(P=0.000<0.01); 血清25-羟基维生素D、总IgE、嗜酸性粒细胞计数均与SCORAD评分相关。结论 青少年及成人特应性皮炎患者的血清25-羟基维生素D水平较健康人明显偏低,且与病情严重程度呈负相关; 总IgE、嗜酸性粒细胞计数与病情严重程度呈正相关。
Abstract:
Objective To investigate the correlation between serum 25-hydroxyvitamin D, total IgE levels and Eosinophil counts with severity of atopic dermatitis(AD)in patients aged 14 to 55.Methods SCORAD was initially done to assess severity in 112 cases of AD patients.Serum 25-hydroxyvitamin D, total IgE levels and Eosinophil counts were detected in patients and 70 healthy volunteers.Results The serum 25-hydroxyvitamin D levels of patients [(20.42±6.96)ng/mL] were significantly lower than healthy controls [(28.68±7.85)ng/mL]( P=0.000<0.01).Compared to patients with mild and moderate severity [(21.62±6.70)ng/mL], the serum 25-hydroxyvitamin D level in severe patients [(18.93±7.06)ng/mL] was significantly lower(P=0.041<0.05).The total IgE levels in patients with high severity [(5 184.08±7 533.82)IU/mL] were significantly higher than patients with mild and moderate severity [(1 075.07±1 777.37)IU/mL](P=0.000<0.01).The proportion of elevated eosinophil counts was significantly higher in severe patients(37/50)than in mild and moderate patients(17/62)(P=0.000<0.01).Serum 25-hydroxyvitamin D, total IgE levels and Eosinophil counts were correlated with SCORAD score.Conclusion Compared with healthy controls, AD patients are have lower serum 25-hydroxyvitamin D levels significantly, and negatively correlated with the AD severity.Both total IgE level and Eosinophil counts were positively correlated with AD severity respectively.
