摘要:
目的 提高临床皮肤科医生对外阴汗管瘤的认识,减少临床工作中的误诊、漏诊。方法 回顾性分析2015年6月-2019年12月本院临床诊治过程中误诊的12例外阴汗管瘤患者临床表现、病理特征、误诊原因、误诊疾病等资料,并进行文献复习。结果 12例外阴汗管瘤主要分布于双侧大阴唇,皮损为多发皮色、褐色、棕红色芝麻至绿豆大小中等硬度孤立的半球形丘疹,均有不同程度瘙痒。病理表现为多数嗜碱性上皮细胞聚集成的小团块、导管及小囊腔,导管和囊腔均有两层上皮组成,部分细胞条索呈蝌蚪状。临床误诊为扁平苔藓、鲍温样丘疹病、Fox-Fordyce、皮脂腺异位、淋巴管瘤、尖锐湿疣、顶泌汗腺汗囊瘤等疾病。结论 外阴部位的汗管瘤临床较为罕见,临床医生缺乏对本病的认识导致误诊、漏诊,临床上如遇外阴部位的丘疹应想到本病的可能,行皮肤病理检查可以及早明确诊断,以避免误诊、漏诊。
Abstract:
Objective To improve the knowledge of clinical dermatologists on syringoma of vulva and reduce misdiagnosis and missed diagnosis in clinical work.Methods The clinical manifestations,histopathology features,causes and diseases of 12 misdiagnosed patients in our hospital from June 2015 to December 2019 were analyzed retrospectively,and the literature was reviewed.Results Twelve cases of syringoma of the vulva were mainly distributed in the hemispherical papules with multiple skin color,brown and brownish red on both sides of the labia magna,which are solitary,sesame to mung bean sized,and moderately hard,and have varying degrees of pruritus.Histopathologic examination revealed most basophil cells were aggregated into small clumps,ducts and sacs.The ducts and sacs were composed of two layers of epithelium,some of which were tadpole like.It was easy to be misdiagnosed as lichen planus,Bowenoid papulosis,Fox-Fordyce,ectopia of sebaceous gland,lymphangioma,condyloma acuminatum,apocrine gland cystoma and so on.Conclusion The clinical diagnosis of syringoma in vulva is relatively rare.The lack of understanding of this disease by clinical dermatologists leads to misdiagnosis and missed diagnosis.In clinical,if there is a papule in vulva,we should think about the possibility of this disease.The skin histopathologic examination can make a clear diagnosis as soon as possible to avoid misdiagnosis and missed diagnosis.
