目的 研究连翘苷对肛周脓肿大鼠创面血管新生的促进作用。方法 60只SD雄性大鼠,随机选取10只作为空白组,其余建模成功后随机分为对照组(10只)、连翘苷低剂量组(10只)、连翘苷高剂量组(11只)、左氧氟沙星组(11只)。连翘苷低剂量组、高剂量组分别采用80、160 mg/(kg·d)连翘苷灌胃,左氧氟沙星组采用左氧氟沙星胶囊63 mg/(kg·d)灌胃,空白组与对照组均给予等量0.50%羧甲基纤维素钠溶液灌胃。早晚分2次干预,连续14 d。末次给药后4 h经创面脓液病原菌培养检测大肠埃希菌菌落数; 取材后脱颈处死大鼠,ELISA法检测创面组织肿瘤坏死因子-α(TNF-α)、白介素-1β(IL-1β)、白介素-6(IL-6)水平,HE染色观察各组创面组织病理变化,计数成纤维细胞数量; 免疫组织化学染色计数微血管密度(MVD)及成纤维细胞数量; 免疫组织化学染色观察创面组织磷酸化Janus蛋白酪氨酸激酶2(p-JAK2)、磷酸化信号转导及转录激活子3(p-STAT3)、血管内皮生长因子(VEGF)蛋白表达情况。结果 对照组、连翘苷低剂量组、连翘苷高剂量组、左氧氟沙星组、空白组大肠埃希菌菌落数依次减少(P<0.05),TNF-α、IL-1β、IL-6水平依次降低(P<0.05); 空白组创面组织病理结果基本正常; 对照组表皮层增厚,聚集大量淋巴细胞,组织水肿严重; 连翘苷低剂量组、高剂量组、左氧氟沙星组干预后组织病变均减轻,其中左氧氟沙星组改善最为明显,连翘苷高剂量组次之,最后为连翘苷低剂量组; 空白组、对照组、连翘苷低剂量组、连翘苷高剂量组、左氧氟沙星组成纤维细胞数量依次增加(P<0.05),MVD依次增大(P<0.05),p-JAK2、p-STAT3、VEGF蛋白阳性表达率依次升高(P<0.05)。结论 连翘苷可显著减少肛周脓肿模型大鼠创面大肠埃希菌数量,减轻炎症反应,促进创面血管新生,推测该药物作用机制与激活JAK2/STAT3通路有关。
Objective To study the role of forsythiaside in promoting angiogenesis in wound surface of rats with perianal abscess.Methods Sixty male SD rats were enrolled.Of them,10 were randomly selected as the blank group,and after successful modeling,the remaining 50 were randomly divided into control group(n=10),low-dose forsythin group(n=10),high-dose forsythin group(n=11),and levofloxacin group(n=11).The low-dose and high-dose forsythiaside groups were given 80 mg/(kg·d)and 160 mg/(kg·d)forsythin by gavage,respectively,and the levofloxacin group was given levofloxacin capsule 63 mg/(kg·d)by gavage.The blank group and control group were given the same amount of 0.50% sodium carboxymethylcellulose solution by gavage.Intervention was performed twice a day in the morning and evening for 14 consecutive days.At 4 h after the last administration,the number of Escherichia coli colonies was detected by the culture of pathogenic bacteria in wound pus.The rats were sacrificed at 4 h after the last administration,and the levels of TNF-α,IL-1β,and IL-6 were detected by ELISA.HE staining was used to observe the pathological changes of wound tissues and count the number of fibroblasts in each group.Immunohistochemical staining was used to count the microvessel density(MVD)and the number of fibroblasts.Immunohistochemical staining was used to observe the expression of p-JAK2,p-STAT3 and VEGF in wound tissue.Results The number of Escherichia coli colonies and the levels of TNF-α,IL-1β and IL-6 in the control group,low-dose forsythin group,high-dose forsythin group,levofloxacin group,and blank group were decreased in sequence(P<0.05).The histopathological results of the wounds in the blank group were basically normal.The epidermal layer of the control group was thickened,a large number of lymphocytes were accumulated,and the tissue edema was serious.The low-dose forsythin group,high-dose forsythin group,and levofloxacin group all had reduced tissue lesions after intervention.Among them,the improvement in the levofloxacin group was obvious,followed by the high-dose forsythin group,and finally the low-dose forsythin group.The number of fibroblasts,MVD,p-JAK2,p-STAT3 and VEGF positive expression rate in the control group,low-dose forsythin group,high-dose forsythin group,levofloxacin group,and blank group were increased in sequence(P<0.05).Conclusion Forsythin can significantly reduce the number of Escherichia coli in the wound of perianal abscess model rats,reduce inflammation,and promote wound angiogenesis.It is speculated that the drug's mechanism of action is related to the activation of JAK2/STAT3 pathway.
