摘要:
目的 评价中重度儿童特应性皮炎(atopic dermatitis,AD)患者的神经认知功能。方法 选取6~12岁中重度AD患者35例,并匹配了相同年龄、性别的健康对照组,采用NICHQ范德比尔特量表(父母版)进行认知功能评定,采用SCORAD积分评价AD儿童患者病情严重度。结果 35例儿童AD患者中有1例符合注意缺陷多动障碍(ADHD)的诊断,健康对照组无符合标准者。AD儿童患者中具有ADHD相关症状者(如:注意力不集中、多动)的比例较健康对照组明显高,差异均有统计学意义(P<0.05)。AD儿童患者病情严重度SCORAD积分与ADHD核心症状积分(TSS)无相关性(P>0.05); 主观症状(瘙痒与睡眠障碍)积分与TSS之间呈正相关(P<0.05)。结论 6~12岁中重度儿童AD患者存在一些注意力不集中、多动等相关行为异常表现。理解儿童AD患者的神经认知功能异常,有助于增强AD的治疗策略和长期的疾病管理。
Abstract:
Objective To assess the neurocognitive function of children with moderate/severe atopic dermatitis(AD).Methods Thirty five children of 6~12 years old diagnosed with moderate/severe AD participated in this study,and compared with age-/sex-matched healthy controls.The cognitive function was assessed by Parental Vanderbilt questionnaire,and disease severity was assessed by the Scoring of Atopic Dermatitis(SCORAD).Results One case was diagnosed with ADHD among 35 children with AD,while there was none in the healthy control.The proportion of children with ADHD related symptoms(inattention,hyperactivity)in AD group was significantly higher than that in healthy control group,and the difference was statistically significant(P<0.05).There was no correlation between SCORAD score and total score of core symptoms(TSS)(P>0.05); there was a positive correlation between score of subjective symptoms(pruritus and sleep disorder)and TSS(P<0.05).Conclusion There are some abnormal behaviors such as inattention and hyperactivity in 6~12-year-old children with AD.Better understanding of the abnormal neurocognitive function in children with AD may help with the treatment strategies of the disease and long term disease management.
