摘要:
目的 探讨甲真菌病与甲银屑病病甲皮肤镜图像特征差异。方法 回顾性分析本院2017年1月-2019年9月收治的甲真菌病与甲银屑病病患者各256例临床资料,分析病甲皮肤镜图像特征和临床亚型资料,比较甲真菌病与甲银屑病病甲皮肤镜图像特征。结果 ①甲银屑病和甲真菌病患者一般资料比较差异无统计学意义(P>0.05); ②远端甲下型甲真菌病患者边缘锯齿改变和纵行条纹比例显著高于其他亚型(P<0.05); 全甲型甲真菌病患者锥形角化比例显著高于其他亚型(P<0.05); ③寻常性和关节病型甲银屑病患者油滴改变比例显著高于其他亚型(P<0.05); 脓疱型甲银屑病患者甲床毛细血管扩张比例显著高于其他亚型(P<0.05); ④甲真菌病和甲银屑病共同特征发生率比较差异无统计学意义(P>0.05)。结论 甲真菌病与甲银屑病均存在各自特殊病甲皮肤镜图像特征,但两者在甲板增厚、甲板剥离及趾/指甲变色发生频率方面较为接近。
Abstract:
Objective To investigate the image characteristics differences in patients with onychomycosis and nail psoriasis.Methods Clinical data of 256 patients with onychomycosis and nail psoriasis were retrospectively chosen in the period from January 2017 to September 2019.The image features and clinical subtypes of the dermatoscope of nail were analyzed and the characteristics of nail dermoscopy between onychomycosis and nail psoriasis were compared.Results ①There was no significant difference in the general data between patients with onychomycosis and nail psoriasis(P>0.05).②The proportion of edge jagged changes and vertical stripes in patients with distal subungual onychomycosis were significantly higher than other subtypes(P<0.05).The proportion of keratonization in patients with all nail onychomycosis were significantly higher than other subtypes(P<0.05).③The proportion of oil drop changes in patients with vulgaris and arthropathy psoriasis were significantly higher than other subtypes(P<0.05).The proportion of capillaries expansion in nail bed of patients with pustular psoriasis were significantly higher than other subtypes(P<0.05).④There was no significant difference in the incidence of common characteristics between patients with onychomycosis and nail psoriasis(P>0.05).Conclusion Both onychomycosis and nail psoriasis have their own special nail dermatoscopic features,but they are similar in the frequency of deck thickening,deck peeling and toe / nail discoloration.
