摘要:
患者男,45岁,面部、背部及四肢散在分布的红斑、丘疹4个月余。皮肤科情况:可见面部对称分布的红色斑片,背部、四肢弥漫分布的红色丘疹,丘疹中间凹陷,部分中央坏死,呈瓷白色萎缩斑。皮损组织病理示:表皮轻度角化过度伴表皮萎缩,基底细胞灶性液化变性,真皮浅层见色素失禁。真皮灶性胶原纤维增生。真皮血管壁坏死,管壁和周边浸润中性粒细胞及核尘,并见淋巴细胞和浆细胞浸润。予以甲泼尼龙、羟氯喹、阿司匹林、双嘧达莫等对症治疗后症状好转。
Abstract:
A 45-year-old male presented scattered erythema and papules on his face,back and limbs for more than 4 months.Physical examination showed the facial erythema are symmetrically distributed.The skin of the back and limbs had multiple papular lesions with central depression,central porcelain white atrophy and a peripheral rim of erythema.Pathology showed mild hyperkeratosis of epidermis with epidermal atrophy,focal liquefaction degeneration of basal cells,pigment incontinence in superficial dermis. Dermal focal collagen fibrous hyperplasia. Vascular wall necrosis in dermis,infiltration of neutrophils and nuclear dust in the wall and periphery,and infiltration of lymphocytes and plasma cells.Diagnosis: overlap syndrome with Degos-like lesions.Methylprednisolone,hydroxychloroquine,aspirin and dipyridamole were used successfully.
