摘要:
目的 探究化学发光法(CMIA)检测梅毒特异性抗体的检测策略。方法 以梅毒螺旋体明胶颗粒凝集试验(TPPA)为梅毒特异性抗体确证方法,采用雅培i2000SR全自动化学发光免疫分析仪检测梅毒抗体S/CO值,比较化学发光微粒子免疫分析法(CMIA)检测梅毒抗体的S/CO与TPPA法检测结果的符合率,探讨CMIA检测梅毒抗体的S/CO复检范围,确保CMIA检测梅毒抗体结果的准确性,对TPPA复检结果不一致样本进行免疫印迹(WB)检测。结果 对29 472份血清样本进行梅毒抗体筛查,CMIA检出1 026例阳性,阳性检出率3.48%(1 026/29 472); TPPA法检出891例阳性,阳性检出率3.02%(891/29 472),两者阳性检出率差异无统计学意义(P>0.05),梅毒抗体CMIA S/CO与TPPA法结果符合率比较:CMIA检测阴性样本:0~0.70 S/CO组阴性符合率100%(27 432/27 432); 0.70~1.00 S/CO组阴性符合率99.51%(1 009/1 014); CMIA检测阳性样本:低值1.0~5.0 S/CO组阳性符合率62.09%(208/335),中值5.0~10.0 S/CO组阳性符合率90.08%(118/131),高值≥10.0 S/CO组阳性符合率100%(560/560),随着梅毒抗体浓度的升高,两种方法阳性符合率也逐渐升高,低值组与高值组阳性符合率差异有统计学意义(P<0.05)。145例CMIA S/CO与TPPA 检测结果不一致样本,用免疫印迹法确证与TPPA 结果一致。结论 CMIA检测梅毒抗体S/CO的低值样本,尤其是对于检测值在灰区0.7~1.0 S/CO及1.0~10.0 S/CO之间的样本需做TPPA确证实验进行复检,以确保检测结果的准确性,减少医疗纠纷。
Abstract:
Objective To study the strategies detecting specific antibody to syphilis by chemiluminescence immunoassay(CMIA).Methods The syphilis antibody was detected by both Abbott i2000SR and the TPPA method.S/CO detected by CMIA were compared with results tested by TPPA method.According to the consistency of these two measurement methods,ranges of S/CO for re-examination was determined.In order to assure the accuracy of CMIA,inconsistent results of TPPA methods were validated by western blot.Results The positive rates of syphilis antibody measured by CMIA and TPPA were 3.48%(1 026/29 472)and 3.02%(891/ 29 472),respectively(P>0.05).The negative consistency rates of CMIA and TPPA were 100%(27 432/27 432)for samples with CMIA S/CO of 0~0.7,and 99.52%(1 009/1 014)for samples with CMIA S/CO of 0.7~1.0.The positive consistency rates were 62.09%(208/335)for samples with CMIA S/CO of 1.0~5.0,90.08%(118/131)for samples with CMIA S/CO of 5.0~10.0,and 100%(560/560)for samples with CMIA S/CO of 10.0.Positive consistency rate correlated positively with S/CO.Positive consistency rate differed significantly between low S/CO group and high S/CO group.In 145 samples with inconsistent results,Western blot confirmed the results of TPPA methods.Conclusion Samples with low S/CO,particularly in those with S/CO ranges of 0.7~1.0 and 1.0~10.0 by CMIA,confirmatory test with TPPA method should be performed to ensure the test accuracy,and to reduce medical dispute.
