摘要:
目的 探索黑素细胞痣(melanocytic nevus,MN)皮肤镜模式与其大小、分布部位及组织病理的相关性。方法 收集2018年3月-11月就诊于重庆医科大学附属第一医院皮肤科的126例黑素细胞痣病例,记录和分析面颈部、躯干、四肢的黑素细胞痣皮肤镜模式,同时测量其直径,并行病理活检,分析黑素细胞痣皮肤镜模式与大小、分布部位、组织病理的关系。结果 网状模式、假网状模式及混合模式在不同大小黑素细胞痣中分布差异有统计学意义。其中网状模式、假网状模式大多直径<0.5 cm,混合模式直径多位于1.0~1.5 cm之间。网状模式、假网状模式在各部位分布差异有统计学意义。其中网状模式多见于四肢,本研究中假网状结构均位于面颈部。网状模式和均质模式在不同病理类型黑素细胞痣中分布差异有统计学意义。其中网状模式多见于交界痣,均质模式多见于皮内痣。结论 不同皮肤镜模式的黑素细胞痣在直径大小及人体部位分布上存在一定差异,且对应不同的组织病理学表现。
Abstract:
Objective To determine correlation of the dermoscopic patterns of melanocytic nevus(MN)with size,distribution and histopathology.Methods The medical records of 126 patients with MN who were treated in Department of Dermatology,the First Affiliated Hospital of Chongqing Medical College from March 2018 to November were collected.The dermoscopic patterns of MN in face and neck,trunk,and limbs were recorded and analyzed.In the meantime,the diameter was measured and biopsy was performed.The relationship between dermoscopic patterns,size,distribution and histopathology of MN was analyzed.Results The distribution of reticular pattern,pseudo-network pattern and mixed pattern in different sizes of MN was statistically significant.The diameter of reticular pattern and pseudo-network was mostly <0.5 cm,and the diameter of mixed pattern was mostly between 1.0 cm and 1.5 cm.The distribution of reticular and pseudo-network pattern varied dramatically in different body parts,of which reticular pattern was more common in the limbs and pseudo-network pattern was only observed on the face and neck.The distribution of reticular pattern and homogeneous pattern in different pathological types of MN was statistically significant.Reticular pattern was more common in junctional nevus,and homogenous pattern was more common in intradermal nevus.Conclusion The diameter and distribution of MN in various dermoscopic patterns are diverse,and different dermoscopic patterns correspond to different histopathological manifestations.
