摘要:
目的 探讨系统性红斑狼疮(SLE)并发股骨头坏死(ONFH)的相关危险因素,为预防决策提供依据。方法 应用Meta分析方法对国内外19篇关于SLE并发ONFH危险因素的病例对照研究结果进行综合分析; 运用Review Manager 5.3软件进行异质性检验并计算各危险因素的OR值和95%CI。结果 SLE合并ONFH的危险因素的OR值及95%CI分别是:血管炎3.08(2.38～4.00),激素冲击治疗2.07(1.51～2.85),抗心磷脂抗体1.78(1.04～3.04),血小板减少1.69(1.17～2.43),高血压1.45(1.05～2.01),雷诺现象1.53(1.21～1.93)。结论 SLE患者早期大剂量的使用糖皮质激素治疗为并发股骨头坏死的独立危险因素外,血管炎、激素冲击治疗、抗心磷脂抗体阳性、高血压、雷诺现象、血小板减少症也是SLE并发无菌性股骨头坏死的主要危险因素之一。
Abstract:
Objective To investigate the risk factors for aseptic femoral head necrosis in patients with systemic lupus erythematosus(SLE)and provide the basis for prevention. Methods Meta-analysis was used to evaluate comprehensively the results of 19 case-control studies proformed at home and abroad. The pooled odd ratio and 95% confidence interval of each risk factor were calculated using Review Manager 5.3 software. Results The pooled odd ratio and 95% confidence interval of each risk factor for aseptic femoral head necrosis in SLE patients were was follows:vasculitis 3.08(2.38～4.00),steroid pulse therapy 2.07(1.51～2.85),anti-phospholipid antibodies 1.78(1.04～3.04),thrombocytopenia 1.69(1.17～2.43),hypertension 1.55(1.13～2.13)phenomenon 1.45(1.05～2.01). Conclusion Early high-dose glucocorticoid therapy is an independent risk factor for aseptic femoral head necrosis in SLE patients. Vasculitis,steroid pulse therapy,anti-phospholipid antibodies,hypertension,Raynaud's phenomenon thrombocytopenia are also the main risk factors for developing SLE.
