摘要:
目的 探讨光动力疗法治疗合并HIV感染的尖锐湿疣的治疗效果。方法 选择50例合并HIV感染的尖锐湿疣患者为研究对象,采用随机分组法将50例患者分为对照组(25例)与研究组(25例)。对照组中疣体直径≤5mm者给予局部使用咪喹莫特乳膏治疗,疣体直径>5mm者先用CO2激光灼烧疣体再用咪喹莫特乳膏治疗; 研究组中疣体直径≤5mm者给予光动力学治疗,疣体直径>5mm者先用CO2激光灼烧疣体再给予光动力治疗。完成各组3次疗程并随访半年,比较两组患者尖锐湿疣的治愈、复发情况以及各组的不良反应。结果 研究组较对照组具有较高的治愈率以及较少并发局部皮肤、黏膜的损伤,差异有统计学意义(P<0.05),两组间患者局部皮肤、黏膜的疼痛情况差异有统计学意义(P<0.05),研究组患者疼痛程度较高。结论 光动力疗法治疗合并HIV感染的尖锐湿疣虽局部疼痛较明显但具有较好疗效,皮肤黏膜的损伤小。
Abstract:
Objective To explore the therapeutic effect of photodynamic therapy on condyloma acuminatum combined with HIV. Methods Fifty cases of condyloma acuminatum with HIV were selected and divided into control group(25 cases)and study group(25 cases)by random grouping method. Patients who had the wart diameter no more than 5mm were treated topically with etomidate and miconazole cream in the control group. As the wart diameter was more than 5mm the treatment started with CO2 laser and followed with etomidate and miconazole cream. In the study group, the treatment of photodynamic therapy was used to the patients whose wart diameter was less than 5mm, while for the wart diameter larger than 5mm the treatment had additional CO2 laser to burn the wart body in the first step. The treatment completed with three courses in each group and the follow-up last six months. During the period the therapeutic effect, recurrence and adverse reactions in each group were compared. Results Compared with the control group, the study group had a higher cure rate and less complication of local skin and mucous membrane, and the difference was statistically significant(P<0.05). The pain of the topical skin and mucous membrane was statistically different(P<0.05), and the patients in the study group had a higher degree of pain. Conclusion Photodynamic therapy is effective in treating condyloma acuminatum with HIV, and the injury of skin mucosa is slight, but topical pain is obvious.
