摘要:
患者女,24 岁。发现躯干、四肢白色斑丘疹8年,皮损散在分布,不能自行消退。皮疹处无痤疮及其他炎症性皮肤病史。皮肤科情况:全身皮肤黧黑,颈部、胸背部、四肢散在多发圆形或椭圆形非毛囊性白色丘疹,以后背为重,密集但不融合,直径约3～5mm。表面光滑,境界清楚,无光泽,无脱屑,触之坚实,无压痛。臀部皮损组织病理示:真皮中央胶原纤维纤细,血管周围少许淋巴组织细胞。弹力纤维染色:纤细胶原区弹力纤维消失。诊断为丘疹性弹力纤维离解。
Abstract:
A 24-year-old female presented with white papules on her limbs and trunk for 8 years.These discrete lesions could not resolve spontaneously. She denied history of antecedent acne and other histories of inflammatory skin diseases on the area involved.Her skin is swarthy. Physical examination revealed of non-follicular white papules distributed on the neck,chest,back and limbs. These lesions were 3mm～5 mm in diameter,oval or ellipse,smooth surface, clear boundary, no gloss, no scaling, no tenderness,slightly indurated on palpation and scattered with no coalescance tendencies extremities worsen on the back. Histopathological examination(Verhoeff's Van Gieson elastic fiber staining)shows focal loss or decrease of elastic fibers in the slim layer of collagen fibers in the middle dermis where there may be mild perivascular lymphocytic infiltrate around the blood vessel.A diagnosis of papular elastorrhexis was confirmed.
