摘要:
目的 探讨中性粒细胞/淋巴细胞比值(neutrophil to lymphocyte ratio,NLR)与寻常性银屑病及银屑病关节炎的关系。方法 回顾分析81例寻常性银屑病和17例银屑病关节炎的血常规指标、C反应蛋白(CRP)水平,计算中性粒细胞/淋巴细胞比值及血小板/淋巴细胞比值(platelet to lymphocyte ratio,PLR),并与60例正常人比较。采用Pearson相关分析了解CRP与NLR和PLR的相关性,用Logistic回归分析银屑病关节炎的相关变量,采用受试者工作特征曲线探讨NLR对银屑病关节炎的诊断效能。结果 寻常性银屑病患者血WBC,NEU和NLR较正常对照组升高(P均<0.05),银屑病关节患者血NEU,NLR和PLR较寻常性银屑病患者及正常对照均明显升高(P均<0.05)。Pearson相关分析显示寻常性银屑病患者血NLR与CRP成正相关(P<0.05)。Logistic回归分析结果显示NLR是银屑病关节炎的独立相关变量(P<0.05)。受试者工作特征曲线显示NLR为3.14时对银屑病关节炎的诊断效能最高,其敏感度为93.8%,特异性为76.6%。结论 寻常性银屑病及银屑病关节炎患者NLR较正常人明显升高,银屑病关节炎患者NLR,PLR明显高于寻常性银屑病患者。NLR对银屑病关节炎有较好的诊断效能。
Abstract:
Objective To explore the relationship of neutrophil to lymphocyte ratio(NLR)with psoriasis vulgaris(PsV)and psoriasis arthritis(PsA).Methods A retrospective analysis including 81 PsV patients, 17 PsA patients and 60 normalcontrols was done.The comparing parameters contained blood routine indexes, CRP,NLR and platelet to lymphocyte ratio(PLR).Pearson correlation analysis was used to detect the correlations.Logistic regression analysis was used to determine the variables associated with PsA.Receiver operating characteristic(ROC)curve was applied to ensure the diagnosis efficacy of NLR to PsA.Results WBC, NEU, NLR were significantly higher in PsV patients than in normal controls(P<0.05).NEU, NLR, PLR in PsA patients were significantly higher than in PsV patients and controls(P<0.05).There was a statistically significant correlation between NLR and CRP in PsV patients.Logistic regression analysis showed NLR was an independent related variable of PsA(P<0.05). ROC curve analysis revealed that the NLR value of 3.14 had the optimal diagnosis efficacy with sensitivity and specificity of 93.8% and 76.6%.Conclusion NLR was significantly higher in PsV and PsA patients.NLR, PLR in PsA patients even higher than in PsV patients.NLR had a potential diagnosis efficacy to PsA.
