摘要:
目的分析医院不同科室报告的梅毒病例的临床及实验室依据，评估不同科室梅毒报告质量。方法从传染病直报系统下载广州市医疗机构2012年梅毒病例并抽样进行核查。分析各科室报告的梅毒病例报告准确率以及过度报告率。结果广州市医疗机构梅毒报告准确率总体为43.79%，过度报告率为47.96%，分类误诊报告率为8.25%。梅毒报告准确率在皮肤性病科、妇产科及其他科室分别为69.50%，40.85%和33.69%，皮肤性病科高于其他科室；过度报告率分别为26.95%，50.00%和56.78%，皮肤科最低，以上差异均有统计学意义(P均<0.01)。过度报告的梅毒病例中，妇产科血清RPR≤1∶8者占81.51%，显著高于皮肤科的71.05%及其他科室的73.05%，同时，还发现妇产科病例血清RPR/TPPA双阳性率25.70%，显著低于皮肤性病科的43.86%及其他科室的42.79%。结论皮肤性病科梅毒报告质量好于其他科室，应加强非皮肤科医生的梅毒诊断及疫情报告培训，或者实行梅毒诊疗及报告归口到皮肤性病科管理，同时，修改完善我国传染病报卡中梅毒报告病例的信息，以提高梅毒疫情报告质量。
Abstract:
ObjectiveTo evaluate the quality of syphilis testing from different departments in Guangzhou medical institutes according to clinic and laboratory evidence.MethodsData of syphilis cases reported by Guangzhou medical institutes during 2012 were downloaded from unified infectious reporting network and reviewed by welltrained physician.Proportion of both accurate reporting and over reporting were calculated.ResultsIn general,rate of accurate case reporting was 43.79%.Rate of syphilis overreporting was 47.96%，and rate of misdiagnose was 8.25%.The rate of accurate case reporting for department of dermatology,obstetrics & gynecology and the others department was 69.50%,40.85%,33.69% respectively,and significant difference was observed (P<0.01).Rate of overreporting from departments mentioned above was 26.95%,50.00%,56.78% respectively.Of the overreported syphilis cases,the proportion of patients whose RPR titer equal or less than 1∶8 in obstetrics & gynecology was 81.51%,which was higher than that from department of dermatology(71.05%) and other departments(73.05%).Meanwhile,the proportion of patients who tested positive for both RPR and TPPA was 25.70%,which was lower than that from department of dermatology(43.86%) and other departments(42.79%).ConclusionThe quality of syphilis testing was significant higher in department of dermatology.The medical institutes should take steps to enhance and expand syphilis knowledge training for nondermatologist.Patients with syphilis should be referred to department of dermatology for diagnose and case reporting,and the ongoing surveillance system for syphilis case reporting should be improved as well for the enhancement of efficiency.
