摘要:
目的 探讨引起系统性红斑狼疮(SLE)患者伴发感染的易感因素,为临床防治SLE患者伴发感染提供理论依据。方法 收集2005年1月-2012年10月本科住院的427例SLE患者,通过单因素和多因素Logistic回归方法对伴发感染的SLE患者的临床资料等进行分析。结果 SLE患者伴发感染发生率为51.52%(220/427); 易感因素主要有补体缺陷和免疫异常、年龄、病情活动、合并有基础性疾病、长期大剂量糖皮质激素治疗等。年龄、应用广谱抗生素、激素冲击治疗、住院时间延长、病情活动性与感染的发生密切相关(P<0.05)。结论 SLE患者易伴发感染,这也是导致患者多脏器功能衰竭甚至死亡的重要原因。补体缺陷和免疫异常、病情活动、合并有基础疾病、特殊治疗方法及不合理应用抗生素是伴发感染的独立危险因素。
Abstract:
Objective To explore the predisposing factors resulting in the infection of systemic lupus erythematosus(SLE)and provide the theoretical proof for preventing and treating the infection of patients with SLE.Methods Data from four hundred and twenty-seven patients with SLE in the department of dermatology in our hospital were collected from January,2005 to October,2012.Clinical data from the inpatients with SLE accompanied by infection were analyzed by single factor and multiple factor logistic regression.Results The rate of infection in SLE patients was 51.52%(220/427).The main predisposing factors included complement dificiency,immunologic abnormality,age,pathogenetic condition,accompanied by fundamental disease,use of long-time with large dose of corticosteroids by single factor analysis.The analysis Results By multiple factor logistic regression showed the age,use of broad-spectrum antibiotics,impulsing treatment with corticosteroids,delay of length of stay and the activity of pathogenetic condition correlated closely with the infection of the patients with SLE(P<0.05).Conclusion The patients with SLE can be easily accompanied by the infection.The infection was the most important cause resulting in multiorgan dyfunction and even death.Complement dificiency,immunologic abnormality,age,pathogenetic condition,accompanied by fundamental disease,particular treatment and the unreasonable use of antibiotic were the independent dangerous factors accompanied by infection.
