摘要:
患者女,21岁。因颈部淋巴结肿大,自行口服网上所购中药“七络丹”8个月,双手足丘疹、颜面颈部躯干色素沉着5个月。查体:双手足见多个密集分布的角化性丘疹,约小米粒大小; 颜面、颈部、躯干见大片棕褐色色素沉着,界不清。皮损组织病理检查示:表皮角化过度,真皮浅层慢性炎症细胞浸润和小血管扩张。尿砷22.10μmol/L。诊断:中药致慢性砷中毒。经硫代硫酸钠治疗后,丘疹和色素沉着明显减退。
Abstract:
A 21-year-old girl had swollen lymph nodes on neck and had taken formulated herbal medicine for 8 months. She had papules on the both hands and feet, and hyperpigmentation on the face, neck and trunk for 5 months. Physical examination indicated dense millet-sized keratotic papules on the both hands and feet. Hyperpigmentation with undefined borders could be seen on the face, neck and trunk. The histopathological examination showed epidermal hyperkeratosis, dermal inflammatory cell infiltration and dilated blood vessels. The arsenic concentration in urine was 22.10μmol/L. The patient was diagnosed with chronic arsenic poisoning caused by Chinese medicine. The hyperpigmentation and papules were improved following sodium thiosulfate treatment.
